The manufacturer of the cholesterol-lowering drug Crestor has stopped marketing the drug after it was found to contain a “black box warning.” The “black box” has been a recurring concern for patients taking a “black box” drug. However, in December, the FDA said Crestor was now “safely available for use by patients in the US who are taking a generic form of the drug.”
“Crestor, the active ingredient in Crestor, is a statin drug,” said Dr. Scott J. Hahn, a clinical professor of clinical pharmacology at the Mayo Clinic and a professor of pharmacy at Case Western Reserve University. “When you take a statin drug, it increases your LDL cholesterol level, and it’s a good thing it has a black box warning.”
The warning was first seen on Nov. 27, 2005, after an FDA advisory panel held a hearing for Dr. David B. Hoehr, director of the Division of Endocrinology at Mayo Clinic. It was later found to contain a black box warning. The FDA had warned Crestor “not to use a statin drug,” but the panel did not consider it safe.
The FDA had already warned Crestor, which was approved as a cholesterol-lowering drug, not to use a generic form of the drug. But the panel agreed that there was insufficient evidence to conclude that the drug caused the increased LDL cholesterol.
The warning was also first made public in 2006, in the wake of a major medical study that found a huge increase in the risk of heart attack and stroke. The warning came after the FDA found the drug contained a black box warning. In the same year, FDA issued a safety notice stating that “the risk of coronary heart disease is not increased by using a statin drug that has been approved for use by a patient in the US.”
According to the FDA, the risk of heart attack and stroke is “not increased by using a statin drug that has been approved for use by a patient in the US.”
The FDA added that the FDA “has never approved a drug that has a black box warning to protect patients from heart disease, stroke, or other related health problems.”
“The FDA is aware that there is a black box warning for Crestor that has been identified by the FDA in connection with the use of a generic form of the drug,” said FDA Chairman Mark C. Rosen, who has been involved in the FDA approval process.
“We are aware of the potential risks associated with using a generic drug and are aware of the concerns raised by patients and other health care providers that may be relevant to our safety recommendations.”
The FDA said it is aware of the “black box warning” of Crestor that was first identified on Nov. 27, 2005, in an alert to be sent to patients who are taking a generic form of the drug. In addition, the FDA noted that the warning was based on “a study of people with high cholesterol who were taking the drug at a high dose for several years.”
In response to the FDA’s announcement, Merck & Co. began promoting Crestor in 2007.
“In a recent safety study, we discovered that there is an increased risk of heart attack, stroke, and other related health problems in patients using a generic form of Crestor. We continue to evaluate this risk to assess whether other medications are safe,” said Dr. Mark D. Merck, vice president of Merck’s drug division. “We’re pleased to see that the FDA has taken steps to clarify the safety warning and has issued a follow-up safety communication to patients.”
According to Merck, “We have received ongoing communication from patients regarding the safety of Crestor.
